Pilot paperwork
Three things have to be on file before you can be assigned a trip. Download the first two, fill them in, and send all three back using the form at the bottom of this page.
1. Acrisure pilot form
The insurance qualification form. It records your certificates, ratings, medical, and your time in type. Complete every field — an incomplete form cannot be submitted to the underwriter.
Fillable PDF. Open it in Preview or Acrobat, type into it, and save before uploading.
2. Form W-9
Required because you are paid as an independent contractor. Use your legal name on line 1, and the name of your company on line 2 only if you invoice through one.

3. Banking details
So you can be paid electronically. Upload a voided check, or a letter from your bank showing the account holder name, routing number and account number.
Please do not type your account number into an email or a web page. A photograph of a voided check carries the same information and is the safer way to send it.

Attach all three below. The connection is encrypted, and the files go straight to the flight department — they are not shared with anyone else.
